# Get Patient Chart

> Get basic facts from a patient's chart: name, date of birth, sex, MRN, and whether the patient is active or inactive at the practice.

Get the patient ID from `POST /patient/match`.

**Rate limit:** 60 requests per minute per practice, shared across all API keys calling it. Requests over the limit get a `429`.
